OBJECTIVE The Market Health Services Director role serves as the market lead for the health services team and clinical operations, directly reports to the Market Executive Director and has dotted line reporting to the Regional Health Services Director. This role also collaborates closely with the Market Medical Director. The Health Services Director (HSD) is a part of the Market Leadership Team (MLT) which ensures all roles in the market execute across the Landmark value levers. This role executes and oversees clinical operations, case management and social work, market strategic initiatives, process standardization, medical supply management, HEDIS quality metric management, outcome performance, and identifies process improvement opportunities to ensure the highest quality of care is given to our patients. The Health Services Director manages the Health Services Managers (HSMs) and Social Work Supervisor(s) who oversee the health services team members:
nurse care managers, social workers, health care ambassadors, transition of care coordinator, and dietitian. While overseeing the day-to-day clinical operations of the market, the HSD maintains focus on high quality care coordination and transitions of care, patient satisfaction, achieving health plan contracted outcomes, strategic management using data and reports, patient engagement and retention, risk identification and utilization management. The Health Services Director maintains relationships with targeted contracted health plan(s)/Independent Practice Association(s) (IPA's) hospice, and home health agency leadership teams, health plan case management, vendors (e.g., DME companies, Labs, mobile diagnostic companies), health care facilities, community-based providers and community organizations in the market. Responsibilities o Collaborate closely with the Market Medical Director and Market Executive Director to ensure the market executes across the Landmark value levers, meets corporate and market initiatives, and meets performance standards for clinical and quality outcomes o Collaborate with members of the Market Leadership Team to implement new strategies, process improvement, change to policies, and local or national pilots o Ensure market is meeting Landmark and contracted health plan/IPA expectations including case management standards for NCQA Delegation/non-delegation, utilization management, risk management, quality metrics, and patient satisfaction o Manage and directly oversee Health Services Team:
including Health Services Managers, Social Worker Supervisor(s), nurse care managers, social workers, healthcare ambassadors, dietitian, and transition of care coordinator. For direct reports (manager/supervisor) this includes conducting regular 1:
1s, proactively providing performance feedback, performing quarterly performance reviews, and supporting staff in career path discussions and development o Responsible for the interviewing, hiring, and retention of the health services team and overseeing onboarding and on-going education o Oversee market medication and supply closet, including but not limited to managing CLIA Waiver, supply and medication inventory, medical waste, and equipment distribution o Analyze data and reports to ensure clinical team and processes are working effectively and market is achieving high quality outcomes and goals o Develop and maintain relationships with targeted leadership teams at contracted health plan(s)/IPA's, hospice, and home health agencies, health plan case management, vendors (i.e. lab companies, mobile diagnostic companies, DME suppliers, medical supplies, etc), health care facilities, community-based providers and community organizations in the market region o Advocate for team needs and solutions, and ensure tools and available systems are optimized to support the success of the team o Provide clinical and operational education to the health services team and market interdisciplinary teams on a regular basis o Manage market complaint and grievance processes to ensure proper reporting and resolution. Evaluate trends and implement process improvement initiatives o Manage external Medical Record requests o Lead or attend market/HSD/National meetings, as required o Collaborate with Regional HSD and cross-market HSD's to achieve the goals of the Health Services Department o Collaborate with Market Medical Director on market clinical operations, opportunities to enhance clinical care and services, manage quality metric program(s), develop educational materials, provide team education, and obtain expected clinical outcomes o Ensure compliance with Landmark Policy and Procedure, State and Federal Regulations, Health Services staff scope of practice standards, and NCQA standards (market dependent) o Other duties as assigned by Market Executive Director or Regional Health Services Director Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. RN License must be current, active, unrestricted and unencumbered, BSN preferred o 3-5 years of increasing supervisory or management experience in a health care setting o 5
years of clinical practice in a hospital, home care, hospice, clinic, or nursing home setting required o Case Management experience and certification strongly preferred o Utilization, Quality, Disease state management experience preferred, but not required o Electronic Medical Record documentation experience required o Computer skills:
internet navigation, Microsoft Office - Outlook, Word and Excel required o Experience working with a geriatric population and in a risk-based environment is desired o Advanced interpersonal and telephonic communication skills o Strong organizational skills o Adaptable, flexible, and able to maintain a positive attitude during change in process, practice or policy o Access to reliable transportation required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy Full COVID-19 vaccination is an essential requirement of this role. Salary Range:
$150K -- $200K
Minimum Qualification
Allied Health Professionals, Healthcare ManagementEstimated Salary: $20 to $28 per hour based on qualifications.
