On Call Nurse-WEEKENDS Location:

Summary:
Provide direct and non-direct nursing care to hospice patients.
Works under direct supervision of Clinical Supervisor/Director.
Independent in the application of advanced nursing knowledge and skills, and manages complex clinical situations for which he/she is responsible.
This position triages, makes after hours patient visits, performs admission, provides discharge visits including pronouncing death.
Salary PLUS $0.
55 per mile.
Education requirement:
Licensed in the state of Michigan as a Registered Nurse.
Experience:
Hospice experience preferred but not required.
Has practiced as a registered nurse the last 3 years.
Responsibilities:
Collects patient data from call center and logs call into patient chart call log.
Triages call with patient and or caregiver Makes visit following algorithm provided and as directed Admissions Discharge visits Regular visits as assigned Emails all staff at conclusion of each day any pertinent information and care provided for the day Shift begins Friday 5:
00 pm until Monday 9:
00 am in accordance with 24- hour care delivery Attends monthly staff meetings Other duties as assigned
